An International Classification of Diseases 9 Revision Clinical Modification (ICD-9-CM) is a Medical Coding Dictionary that was used for coding and classifying morbidity data from the inpatient/outpatient records, physician offices, and National Center for Health Statistics (NCHS) surveys.
